We're pleased that you're considering entrusting New Image Medical Center with your healthcare needs. Our streamlined process makes it fast and easy to join:

1. Call the New Image Medical Center at 732 923-1777. Please have your medical insurance card available when you call.

2. We will answer any questions you have about New Image Medical Center. Many of your questions may be answered by reading the "Frequently Asked Questions" page. We will also schedule your first appointment.

3. Download, print out and complete the New Patient Registration Form, which provides us with important information [FORM 1].

4. Read the New Image Notice of Privacy Practices & Office Policy, then download, print out and sign the Acknowledgement Form [FORM 2]. This indicates that you have read the document and understand your rights and our policies. If you have any questions about the form, please contact our office and we'll answer your questions.

5. Download, print and complete the Authorization for your current health care provider to release your medical records to New Image Medical Center form and send it to your current physician [FORM 3]. This will authorize that doctor's office to forward your medical information to New Image Medical Center.

6. If you wish to authorize any other individuals to have access to your medical records (i.e. spouses, parents, etc.), download, print and sign the Authorization of Use and Disclosure of Protected Health Information form
[FORM 4].

When you arrive for your first appointment with New Image Medical Center, please bring your completed forms and medical insurance card(s). It's best to arrive about 15 minutes early so we can review your forms prior to your appointment.
